Finally managed to get my car registered in Islamabad after 4 hours of khawari. Since I haven't seen the process listed anywhere else, I'm going to describe it here so others may benefit, I wasn't able to find this information anywhere online and running around between windows and getting photocopies/affidavits took at least 2 hours of my time.
Obtain the registration form. You can either download it here (the last 2 pages of this form) or get it for 70 rupees from the form booth in the excise office. They give you a file folder as well and these guys prefer to receive all documents within that folder.

Fill your form. You also need to attach:
- Your CNIC copy
- Car's sale documents (invoice, sales certificate)
- Copy of Covid vaccination certificate
Drive to the inspection point around the back. Exact location here. A guy will verify the chassis no./engine no. and stamp and sign your form.
Some additional things to consider (if applicable). If the address on CNIC is not Islamabad, attach a copy of your rental agreement along with owner's CNIC copy and a utility bill. Plus there was also another random requirement for an undertaking on a stamp paper that in case of increase in taxes, you will be responsible for paying the difference. This was an on the spot rule they devised saying that they don't know if applicable taxes are increasing. I was going to head to F8 to get this made but luckily there was a guy parked in a black wagon R in front of the office doing this for everyone for 300 rupees each. He must have made a killing I'm sure. This might not apply in normal days.
Take your documents to "Room No. 2" where some afsar will sign off. This guy inquired about my address and asked me questions like rent kitna hai (lol). Once satisfied that I'm an Islamabad resident he signed off.
Get your biometric done. There is a biometric counter in the NBP bank area. To save time you can get it done in advance before going from any Nadra e-Sahulat facility.
Take the whole folder with the biometric slip to the last cabin in the area where there is a 'Form F' line. These guys will generate Form F and a bank payment challan.
Deposit payment at the NBP cash counter on the premises. Take cash with you beforehand to save time, registration cost was calculated at 2% of total invoice (inclusive of WHT and freight). In my case at the old Sportage Alpha price the total including other charges was PKR 98,320.
Add the challan receipt to your folder and take it the last cabin again to the 'Aik number counter' to generate a final receipt. You can write down or tell this guy your preferred numbers, he will check if available and assign it to you. You will need to have cash at hand for an additional payment of PKR 2,262 as number plate charges.
Secondly, I am not sure if this was right or not, he kept the whole folder. This included my original invoice and sales certificate from KIA. I have copies but I don't know if this will affect me later on if selling the car. He gave the final receipt on which the plate number of my car was mentioned.
There is a number plate counter at the back of the building where they have most of the number plates already printed and available. Show your receipt and collect your plates. If they don't have it you'll have to come back at a later stage, but your car is already registered and you can use the number given on the receipt.
Smart card will apparently be delivered later, I'll get a text message on my phone number to collect. Meanwhile the final receipt is my registration document.
Hope this helps as it was an utterly confusing and a very stressful process for something that could easily be done online or through a one window operation. If I had known all the steps I would have saved significant time. Also Zong network does not work in the entire excise department areas, so if you have Zong like me be prepared for no connectivity. The place opens at 9 am and the ideal scenario is to get to the inspection point by 9. You can ignore all the agents and touts in the area and do this whole thing yourself.
